it seems your combo box data sources are showing error because items column doesnt have valid formula to get the items you can use formula like =linstname.columnname, so that when you run it will get value from that particular column and display in combo box Here poweraps is my list name and i am refering title column values in combo box

Thank you for your reply. I looked more into creating a form using PowerApps and followed http://simplesolutions365.com/how-to-create-a-custom-form-and-workflow-using-powerapps-and-flow/ Form is created, however when I try to enter information in the form and hit save, it gives me an error message "there was a problem saving your changes. the data source may be invalid".
