How do I make my C# Windows Forms application report stats?

Question

I've created an application that is pretty widely used and would like to get an idea of just how many people use it and how often. How can I implement something that will do that?

Make an entry of each and every user, on the initial load. There will be a default Form naming like Form1 or an MDF form. In that form load you can write each and every entry..
